Are you looking for a pediatrician closer to home?

Or perhaps you are new to the area and are looking for a great pediatric practice?

Whatever the reason, we are happy to have you join our family!

Switching is as easy as 1-2-3!

1) Browse through our Resources page to make sure you agree with our office policies. Please pay particular attention to our vaccine policy as we do require all of our patients to be fully vaccinated (with the exception of flu and COVID vaccines which are recommended but not required). You will also find a list of insurances accepted.

2) Get a copy of your child’s immunization record (if you have that readily available, we will gladly take a copy of it prior to your child’s first appointment). For anyone unable to get a complete immunization record, try asking your child’s school for what they have on file for your child - oftentimes they will have a complete record.

3) Fill out an “Authorization for Release of Medical Records” form in order to transfer your child’s records from his/her previous pediatric practice. For any patients with more complex medical conditions (sees several specialists, chronic conditions, on ADHD medications), we will need to get those records prior to your child’s first appointment.
